New Patients

Please let us know when you make this appointment if you would prefer to print this form from our Patient Portal in which case we will assign you a Validation Code. You will then have 24 hours to log into the Valant Patient Portal page and set up your account with this code.

Once you have set up your account, please print the “Patient Information” form, complete it and bring it with you to your first appointment.
